I want to put two ads above the fold. Can I use the exact same code twice? Just want to make sure this is not an adsense violation.

Adsense used to give the opportunity to use unlimited number of ad blocks under an account. but recently they limited the number of blocks per account. So you should use the same code twice if needed.

You can put 3 ads above the fold. No need to create a new block unless it's a different size, and/or you want to track each ad like that.

When did they put a limit on the number of ad blocks you could create?

In fact, it would be silly to limit it. Logically speaking.

What defines a bunch? Google only allows a total of 3. Hardly a bunch.
3 above the fold is doable. But remember, it's not "above the fold" by
    itself.

    Too many people misquote that.

The real issue if people having to scroll down past ads to find any content.
That's the issue. If you put 3 ads above the fold, and content starts there
too, no problem. Here's just an example:
One 728 ad on top, two 336 blocks, vertical, on right side bar.
(or smaller) Lots of large sites do this.
Content starts within the first 91 pixels, or thereabouts.

People need to stop listening to a guy who knows a guy who
    heard a guy...
