Well, with 40k-60k uniques a month, I am on a dedicated server. I used to self host, back in the day, but that eventually became silly when the cloud became much more popular.

I've used e-Junkie for 10+ years. While I'm currently looking for a new provider, it's merely because their shopping cart interface with Analytics is not tracking well.
