How to do Facebook Optin Pages like this?
Anyone know what app / site this guy would have used to be able to create a tab and an optin page like this?

It's 4 separate images (including the optin box) but with the ability for people to enter their details on the page.
I clicked on one of his ads out of curiosity and saw the page style...I know sending FB ads to a FB page makes for a cheaper CPC vs sending to an external squeeze page...
Anyone know how he would have done something like that? (I'm just getting into the early stages of building a FB page to promote my offline company)
