Theme or plugin for a web form?

I'm looking for a theme or plugin for a web form. I'm looking for a format I can use with my company. I need a form to fill out with all types of various information like:

Location
Tax Rate
Insurance requirements
Schedule duration (enter a start and end date)
and other misc. info including being able to upload a document

that once the form is filled out and submitted it will be emailed to a few different people.

I work for a project management company and I'm trying to streamline our startup process where we can enter all of the project info in one place and it will be sent to all necessary contacts. As of right now, when we are at startup we have to get info together and send it to a scheduler, then get different info to risk management, then get resumes from marketing, etc. It would be nice to fill out all info once and it goes to everyone who needs to look at it.

Any ideas? thanks in advance

    You want Contact Form 7, search for the plugin in your wp admin.

    You can set the certain labels to use and have it sent to a few email addresses.

    There is a bit of a learning curve with this, but generally it is very easy to use. If you have questions about the plugin, please ask.

      I suggest Visual Form Builder (VFB). I have tried Contact Form 7 and several others but VFB is head and shoulders above anything I tried. I bought the Pro version because I had the need to change the form based on conditional statements.

      But I used the free version of VFB for a while before I upgraded. The free version came with tons of features - many were only available in Premium versions. I was amazed at all it would do for free.
