Unique tracking for the same URL's

We have NPS survey forms set up, each score (0 - 10) is on it's own landing page hosted on our website. At the moment we only use these landing pages to collect data from one segment (customer service tickets), but we're looking to open the NPS survey up to our online learning platform, and to our implementation team milestones.

Can anyone suggest how I could set up a unique URL to have a suffix, so it shows which campaign/landing pages the NPS survey was completed on?

I thought at first this may be something we could set up in bitly, but I've never done this before?

    For a similar case, I added a hidden input in the form that is filled automatically, for example customer_service or online_learning_platform or any other etc.... This can be captured automatically based on the referring page. The respondent does not do or see anything and the team gets the additional information for analysis. Hope this helps you
