There are different ways of doing it.

1. Someone can design it in Photoshop/GIMP and send you the source file for you to edit the text, as long as you have the software to do it.

2. Create the graphic and set it as the background in CSS then add the text using HTML.

Make it a CSS image background and use forms if you want people to be able to fill it out....

If you have an image in z-index=0 and you have texts for z-index=1 your texts will be on top of this background image. It's a simple CSS trick.
