Anyone know a plugin that can do a contact firm or how to get a contact form looking like the one on this page

Hey, you can use WordPress › Contact Form 7 « WordPress Plugins - I've created some pretty robust contact forms with it. However for placement and design you will need some coding skills to get it looking like the link you provided.

  For more advanced forms Contact 7 is very useful as you can setup additional fields and tweak settings. But as mentioned you need to be able to play with the CSS to style the form.

I prefer Contact Form 7 for regular contact forms - but I also like the simple convenience of having a mini contact form on a "slide out tab" that the Dynamic WP Contact Form provides. Take a look at: Add a Smooth Slide-Out AJAX Contact Form to Your WordPress Site
