That's nonsense. You cannot "embed" html code into an image...

Forget the terminology that you don't understand and describe in plain English what you want to do and what is the desired end result.

trying to make things clear here...

You mean you want to insert your customized image on a mailchimp opt in box?

I simply don't know the answer, as I only stay with mailchimp for a while. And after that they kicked me out of the system, because I'm doing the whole internet marketing and earn money from home business...

You might want to reconsider using another service if you are also on the same business.

I know only of 2 ways:
        - you learn HTML/CSS
        or
        - you pay somebody to do it

there are tutorials online for this. to put it simply..

1/ in mailchimp download the code for inserting a form on your website (they provide this in one of their menus, took me a while to find).

2/ choose the basic option with no javascript.

3/ strip back all the junk they include with the code so you're just left with the input fields and a submit button.

4/ use a background image for your form using css background-image.

of course all of the above assumes you can code. if not, learn, pay someone, or use mailchimps free Wordpress plugin...
