Wordpress Page Question

Okay everyone....got a question that might be a little bit involved...I am not sure.

The question is this:

How do I add text entry boxes or drop-down boxes on a static wordpress page that will create a database for ordering purposes?

Example:

Name: Text Entry Box Here

Email Address: Text Entry Box Here

Number of Articles: Drop-Down box here

Keywords: Text Entry Box Here (Bigger than ones above and allow multiple lines)

Buy Button

Is this too complex to do via forum chat and need to have it made or is it fairly straight forward?

I tried to find tutorials but was not very successful or did not know the right search words to use.

    Forget about the database and use Contact Form 7. You custom create the fields and you'll get every order in your email.

    Thanks for posting the responses....question for you all. Is there a way that when the person pushes the buy button, the cost is already associated???? Shopping cart style??

    Example:

    Someone buys 5 - 500 articles, when they push the buy button...paypal charges them $25?
