Rate and review plugin for responsive web site

My first post here and I feel the pure energy from all of you people who live this lifestyle. Thanks in advance!!
I am building a community website that has info on the top schools in the area, parks, recreation and attractions. I would like to add a plugin to my site that will allow the people to leave a 1-5 star review of a school or park etc. and as well leave a description or reason why they rated it such. Does anyone know where I can find this plugin or code?
I have searched out a few but they did not have a facebook/twitter login or a recaptcha and anyone could leave a rating....makes me worried about the SQL injection risks.
