    I could be wrong, but it doesn't look like the site is using a plugin for its quiz, looks more like it's custom coded.

    Yep - looks like it's custom coded. If not custom, it's integrated into the theme in a custom manner. You can tell by looking at the code and seeing that all of the CSS is inside of the themes own stylesheet.

    Yep. Not a plugin. The theme is Genesis and the only plugin they're using is Contact Form 7.
