Google Display Network. How to setup Manual Placements for my ads?

Hello,
When I create GDN Campaign I cannot choose whether to use automatic or manual placement approach for my Banner Ads. Please anyone describe me why it is so, and how/where can I do this? May be Google just changed its UI again?
Also another question: when I using default automatic placement approach, I always have lot of impressions and few clicks. I need to get a detailed report by all that impressions, I need to know on which sites impressions have come from and I need to filter some websites which does not get clicks. In general I going to maximize CTR value and then pay for impressions rather then clicks.
