This was happening to me last summer, every time i hit $50 in spendings for the day my ads would stop and I would get this message. I found out later that something was causing a manual site review....

I make a mini-site with wordpress and a few articles...then i make a custom landing page including links in the footer to my about, terms and privacy pages. That works.

In my opinion, CPA (cost per action) is mostly non-wallet out transactions. Such as filling out a lead form etc. I have heard that the conversion rate is about 10 - 11% (for fairly targeted traffic)...

I think the best advice that I had even heard is that content is king. Make good content and your SEO will come much easier as you might have many sites link to you!
