This has nothing at all to do with an update. Google does not have to use the title that you create. They have always tested alternative titles to see what gets more clicks - the one you created or...

Using a + sign won't cause any issue from the point of SEO. If you feel it looks odd then you can simply use a & symbol instead of + sign.
