Make sure you're logged out of any Google accounts when you search for your site. Google personalizes your searches and if it notices you going to one site again and again then it will give that site...

Login to your Adwords Account, then run Google Keyword Tool to Check Your Keyword. I'm sure you'll find different result.
Also check the keyword trend. be sure the keywords doesn't get decreased...

Have you checked your ranking using Scroogle Scraper?
You may not know but there are hundreds of data centers around the world for the google.com domain. Scroogle lists the US results.

Double check your ranking by searching through a proxy like pagewash.com or any other. Your ranking could be in your cache or cookie if you have been searching yourself often. I thought I was #1...

Which one you use? The legacy one or default keyword tool now which is in BETA? In my opinion, the BETA version is more accurate now according to my research and testing.

Google keyword tool shows numbers that are far from being accurate. They are just an estimation. Always use google trends and google keyword tool.
