In the robots.txt file if you have this:

User-agent: *
Disallow: /

That will stop the MAJOR search engines crawling your site, (not the smaller ones, because they dont all use the robots .txt...

If its a problem with the HTML of the form, then you can just run it through the w3c validator, this should pick up and issues you have (although its only going to pick up the HTML), it wont tell you...

HTML 5 / CSS3 is the future, i wouldn't recommend it now because not all browsers support it, but i suppose if you learn CSS2 you will be very close to CSS3 (afterall its not a hige jump),...

Nope as far as i know it doesn't matter, some sites such as: jquery[dot]com, do not have www even though you write www[dot]jquery[dot]com, I guess they edit their .htaccess file to get rid of it....
