Never done it but it's definiately worth the try. I'd setup a split test for your traffic and see how much the survey hurts your conversion rate and whether the long term income makes up for this.

It should only take a few hours at most. If you Max CPC is set too low your Ads won't show.

Are your Ads still showing that they need approval or are you not seeing any impressions?

maybe try lowering it a bit and geting in the 3-4 position... that really is a great cpc you just need to work on optimizing your landing page.
