Generally they get a lot of leads with paid traffic. PPC, PPV and Media buys are the main ones but you have to invest a lot of money before you can make a lot of money.

Depends.... Some networks have some incentive offers (for example maxbounty) and others are 100% non-incentive (for example peerfly).
