the level of difficulty needed really depends on how much spam you get - if you have a massive site you might want to use images or whatever to add an extra layer.

For my site I just coded in a very simple maths puzzle - there are only like 5 different options, so seriously not hard to crack, but it has literally stopped 99% of the spam I get.

This is a really good point. I guess it wouldn't be an issue for many smaller sites, but yeh, if you are getting that volume per day, it bears thinking about.
